» New Patients: If you are a new patient or have not been to our office in the last 3 years please call our office at (630) 355-1355, during business hours, to schedule your appointment. (Please note that we do not accept insurance at this time. For more information please see our FAQ page.)

» Returning Patients: If you have been seen in the last 3 years, please request an appointment below by using our online scheduling tool. When scheduling your appointment, please complete all requested information and indicate the reason for your visit. 

» Walk-ins: We do allow walk-ins for injections only during normal business hours, all other visits may require an appointment.

» No Show/Cancelation Policy: If you need to cancel an appointment, please call our office at (630) 355-1533. No shows or appointments canceled less than 24 hours in advance may be subject to a non-refundable $25 fee per our office policy.
